Print and fill in the registration and medical forms and bring them with your payment to our Sunday clinic at Kaiser High School Gym. Make checks payable to Hawaii Jr. Volleyball Club or HJVB. If you would like to try out the clinic before making a commitment, please let the person at the front desk know.

One-time Cost:
$70 Equipment Fee and Club dry-fast shirt
Annual Cost:
$25 Insurance due each January or upon registration.
Monthly Donation*:
$20 per Clinic or
$68 for the month

* Hawaii Jr. Volleyball Club, Inc. is a 501(c)(3) non-profit corporation. Donations may be tax deductible depending on your personal tax situation.
Tryout for new participants only: $20 – Please fill in registration form and bring it with you when trying out. You will be required to pay all fees above if you decide to return to the clinic. The monthly donation will be determined on the date of return so you will not need to pay for sessions not attended.
